• Cancellation Form: This form lists all possible cancellation options. See your loan note(s) for the options available to you.
• Deferment Form: This form lists all possible deferment options. See your loan notes(s) for the options available to you.
• Forbearance Form: Forbearances are granted, if allowed by loan terms, when you find yourself in financial difficulty.
• Unemployment Deferment Form: This deferment is only available on Perkins loans. Other loans may be eligible for a forbearance if you are unemployed.
• Direct Payment Form: This authorizes Student Financial Services to automatically deduct your monthly payment from your checking or savings account.
• Authorization to Speak Form: Use this form to allow another person to speak with the Student Financial Services about your loans.
• Medical School Loan Deferment/Forbearance Form: This form is also used for M.D. Internship Residency and Fellowship deferment.
