What documentation is necessary for a Name Change?
The Employee Information Form and a copy of the legal document stating the name change are required (Marriage certificate, Court Order or Social Security Card).
Can I have my pay stub or W-2 Form sent to an outside agency (Bank/Accountant)?
Due to the confidentiality of this information we do not fax or mail pay stubs or W-2 forms to outside agencies, or on campus departments.
