Dartmouth has established channels for asking questions about regulatory compliance, seeking guidance about College policies or procedures, or reporting suspected violations of law, policy, or business ethics. Reports may be made anonymously.

Employee Injury Report

All injuries to employees should be documented and reported no matter how minor they may initially appear. This is done to protect both the employee (to ensure prompt receipt of benefits) and the College (to avoid late reporting assessments). The following is the process for reporting work-related injuries to an employee:

  • Employee promptly reports injury to their supervisor
  • If an injury is an emergency getting the employee proper treatment is the priority
  • All injuries should be reported before the end of the shift
  • Injured employee completes the Employee Injury report
  • Supervisor must complete the Supervisor's Investigation report
  • If an employee cannot complete the report the supervisors must complete the Employee Injury report
  • Within 24 hours fax the completed forms to Risk Management at 646-9199
  • Mail the original completed forms to Risk Management at HB 6012
  • Answer all questions as fully as possible but do not delay sending report due to missing info
  • Keep in mind that the State will assess a fine (up to $2,500) against the College if the report is filed late
  • Late reporting assessments will be charged to the reporting department
  • Employees are required to treat with a network provider
  • Employees should follow their doctor's advise and not exceed any limitations that are imposed
  • The law requires the College to provide temporary alternative duties, when appropriate
  • The law requires the employee to accept temporary work that is approved by their doctor

Dartmouth College has a managed care network of providers as part of the workers' compensation program. A list of providers can be obtained through Risk Management.
