Expanded Dartmouth College Employee Health Benefit Reimbursement for
Fitness Memberships
Effective July 1, 2006, benefits-eligible employees and retirees enrolled in
ANY of the Anthem health insurance programs are now eligible
to receive a $200 reimbursement from their medical plan for the expense of
purchasing a membership. To receive this benefit, you must exercise twice
a week in the gym for 12 out of 20 weeks. Additionally, you must complete
a fitness log card and have it initialed by a fitness club staff member each
time you exercise.

steps listed below.
For program information:
- Go to Anthem’s Web site at www.anthem.com.
- Select “New Hampshire” from the Select a State
drop-down menu.
- Click “Enter”.
- Click the “Answers@Anthem” tab at the top of the
Member Home screen.
- Click on “Special Offers” from the bulleted list on the
Answers@Anthem page.
- Click on the “Fitness & Weight Management” tab at the
top of the Special Offers@Anthem page.
- “Fitness Reimbursement Program” is at the bottom of the
Fitness & Weight Management page. You will see buttons for
“More Info” and “Link to Offer”.

- Click “More Info” for detailed information about the
Fitness Reimbursement Program, including requirements and the Anthem
contacts.
- “Link to Offer” will display a list of participating
fitness clubs in New Hampshire.
For the Fitness Reimbursement Program Log Card and the Fitness Reimbursement
Form:
- Go to Anthem’s Web site at www.anthem.com.
- Select “New Hampshire” from the Select a State
drop-down menu.
- Click “Enter”.
- Click the “Answers@Anthem” tab at the top of the
Member Home screen.
- Click on “Download Commonly Requested Forms” from the
bulleted list on the Answers@Anthem page.
- Click on “Fitness Reimbursement Program Log Card”.
- Click on “Fitness Reimbursement Form”.
The Log Card and the Form are also available via links on the HR Forms page.
