Welcome!
As an incoming Dartmouth student, you have pre-matriculation health
requirements to complete before your first term at Dartmouth. These
requirements are presented in both paper and on-line forms. The health
service packet which you will receive through the mail will give you detailed
instructions on which forms may be completed by yourself and those that need to
be completed by others (such as a physician or a health insurance
representative).
The Health Service Mailing
All students entering Fall 2008 should look for a Health Service package
containing brochures, information and form(s) to satisfy immunization
and health
insurance requirements. Please watch for your package to arrive
in a 10 X 13 inch white envelope. We use First Class mail for U.S. addresses
and Air Mail for international addresses.
On-line Forms: Required for all Students
Most of our forms are on-line this year. Please carefully review the
instructions enclosed in the mailing. The instructions will direct you to the
required forms and provide you with the information on how the forms must be
completed.
Access: Your access to on-line forms is through the College
Record System; Banner. How, and when, you gain access to these forms
varies by class. The links below will help connect you to the forms
available. Please note; the on-line forms are NOT printable
forms. These forms must be completed and submitted through the web-site. For a
printable list of immunization requirements please click on the ‘immunization’
link above.
Graduates School of Arts and Sciences, Masters of Arts in Liberal
Studies (MALS), Thayer School of Engineering, and Center for the Evaluative
Clinical Sciences (CECS) may not have access to the on-line system until they
arrive on campus. They should use the link provided below to access
guide-forms for completing requirements using the link indicated
below.

Paper forms for Undergraduates Only:
*Athletic
Physical Form: Undergraduate students who wish to participate in
NCAA or Rugby will be required to complete this form. If you’re not sure
on participation at this time but may consider participating once you are on
campus, we recommend completing the form prior to arriving on campus.
Undergraduates who do not plan on participating do not need to complete this
form.
*Special Form for Minors: Students who will be under the age of
18 on registration day must have their parents/guardian complete the Special
Form for Minors
Note: When you give a form to someone else to complete, such as a doctor or
your health insurance plan representative (see Dartmouth Student Group Health
Plan link), we recommend that you request the form(s) be returned to you when
complete. This allows you to track your forms and prevent others from causing
missed deadlines by sending in paperwork late. Obtaining a personal copy
of the forms/records is recommended as well to bring to campus in case of any
transmittal/mail delays.
