This calculator will provide you with information about your potential treatment outcome based on how your answers to a few questions match those of SPORT participants.

You will be asked some questions, and the answers you provide will result in a graph that compares how you might do with surgery versus how you might do with non-surgical treatment.

Simply click on your diagnosis below. You will be taken to a screen where you will fill out the answers to a few questions about your current physical function relates to your back pain, and about how bothersome your symptoms are to you. Answering these questions and getting your results should only take 2-3 minutes.
