List of Forms to Complete

Once a student has been hired, he/she needs to fill-out the necessary documentation. This documentation must be completed within three days of the student's first day of employment.

I-9 Form (Employment Eligibility Verification)

The Immigration Reform and Control Act of 1986, requires all employers to verify the employment authorization and identity of each person hired, including U.S. citizens. To satisfy the record-keeping requirements of the law, all students employed on campus must complete an Employment Eligibility Verification Form (I-9). The I9 form can be completed with the Student Employment Office. Each student must provide proof of citizenship status by furnishing documentation as listed on the I-9.  Students who have previously been employed on campus should have these forms on file, but it is wise to check with the Payroll Office. All first year and transfer students working on campus will require these forms. International and Canadian students may require other forms as verification. Questions about completing these forms should go to the International Office and the Payroll Office.

W-4 Form (Employee's Withholding Allowance Certificate)

Each student employed by the College must have a current W-4 form on file with the Payroll Office. If a student does not have this information on file, the Payroll Office will use the default of single marital status with zero allowances to process the payroll.  The W4 form can be completed with the Student Employment Office.

Employer's Request for Child Labor Form

No student 16 or 17 years of age shall be employed by an employer unless the employer maintains on file a signed written document from the youth's parent or legal guardian permitting the youth's employment. The NH Employer Request for Child Labor document is to be kept on file in the Payroll office at Dartmouth College.

Work-Study Authorization Form

In the past, SEO has required employers of work-study students to fill-out a Work-Study Authorization form. This is no longer the case. Instead, we ask that you include your Jobnet Number on the student's timesheet. This will ensure your student is being paid through the correct funds. Please contact the Student Employment Office if you do not know or do not have a Jobnet Number.

The Jobnet Job Description Form

The Jobnet Job Description Form is to be filled out by you, the employer. This form is of much importance, whereas it serves not only as an advertising tool and a basis to pay your student worker, it will become part of your own student training manual, and finally it will be used as a tool of measure for student evaluations.

Student Temp Job Form

The Temp Job Service was created to accommodate jobs that don't necessarily fall into the guidelines of Jobnet. These jobs are usually one time only, seasonal or "odd jobs". The purpose of Student Temp Job Service is to give the Dartmouth Community and the Upper Valley a free alternative to advertising for student help. Temp jobs have consisted of shoveling snow, gardening, babysitting, help moving furniture, setting up computers, general office work for a day and the like. Dartmouth students monitor Blitz Bulletin to keep informed of these postings.

Leave Term Work-Study Request

A student must apply for Leave Term Work-Study funds. If you are hiring a work-study student to work during his/her leave term, please encourage the student to fill-out a Leave Term Work-Study Request Form. SEO will inform the student of his/her eligibility.
