If this form is not fully completed and returned by Noon of the 3rd Friday
in October, your organization's account will be frozen until such time as
it is completed. Any organization that does not fully complete the Renewal of
Recognition materials by June 1 of each academic year will lose its status as a
COSO recognized organization.
