Assessment Surveys

Prior to most core values workshops, an assessment survey must be completed by the participant (a self-assessment) and by three to six respondents of the participant's choosing.

A feedback report is given to each participant at the training session. This is an anonymous, confidential summary (prepared by an outside consultant) of the collective perceptions of the respondents. The summary is intended to help the participant understand others' perceptions of their performance and to provide useful information on how to improve.

Assessment Survey Instructions

Participants:

Once you have registered for a session, select and write down (for your records) the names of three to six respondents and ask them to complete feedback surveys for you. Respondents should be people with whom you currently work (or, if you are new to your position, people with whom you have recently worked) who can provide accurate and honest responses to a brief survey. One of your respondents should be your supervisor. The surveys take approximately 10 minutes to complete.

Tell your respondents the name of the workshop(s) you are attending and direct them to this website for instructions.

PLEASE NOTE! Workshop participants MUST do a self-assessment as well. All surveys including your self-assessment, MUST be completed 1 week prior to the session date.

Respondents:

Once you are at the survey site,

  1. Enter the word "Dartmouth" as the ID; and the password "Professionalism" or "Teamwork" or "Feedback" (quotation marks not needed) depending on which Core Values session you are attending. I.e., if you are attending Feedback, you would enter Dartmouth as the ID and Feedback as the password.
  2. Click submit.
  3. Select the workshop participant's name from the left-hand column.
  4. Select the rater's name from the right-hand column.
  5. Click submit.
  6. Complete survey.
  7. Click submit.

ALL surveys must be completed 1 week prior to the session date.
