Claims Reporting

Despite all of your and our best efforts, accidents can and do happen. When they do, it is important to notify the Office of Comprehensive Risk Management as soon as possible so that appropriate resources can be activated, and in those cases where insurance applies, the claim process can be initiated. This page includes forms you can use to collect relevant information when reporting an incident to this office. Please contact our office for further assistance if you have questions.
