Instructions for Obtaining an Undergraduate I-20 or DS-2019 Form
Students requiring a visa status to study in the U.S. must supply complete documentation to the Office of Visa and Immigration Services. After admission to the College, upon receipt of the following documents, the Office of Visa and Immigration Services will issue either a form I-20 or DS-2019, as appropriate and if required. To begin the process, admitted Class of 2011 undergraduate international students should fill out and return the following to the Office of Visa and Immigration Services:
Final Financial Aid Award Letter. Students requesting financial aid must contact the Financial Aid Office for more information.
Statement of Financial Responsibility. Students who have been accepted may use this form as part of the I-20 request process. Students requesting financial aid will need the Financial Aid Award Letter to be able to fill out this form. This form is a promise to pay the tuition &fees itemized on the Financial Aid Award Letter. It must be signed by the payor and notarized (witnessed) by a notary public, Commissioner of Oaths or similar official who is authorized to take oaths.
Supporting Evidence of Financial Responsibility. For whatever amounts are promised on the Statement of Financial Responsibility, supporting documentary evidence must be supplied by the student, e.g. certified copies of bank statements, letters of employment or the like.
Upon receipt of all required documents, the Office of Visa and Immigration Services will issue Form I-20 or DS-2019 and forward them via UPS Express mail to the student.
