Revised NIH forms (PHS398 & PHS2590)

Please note that the NIH released the following revised forms to be used for ALL proposals (paper applications) submitted on or after MAY 10, 2006:

    • PHS398, for new and competing applications
    • PHS2590, for progress report

All forms are available in Word or PDF format on the NIH website at: http://grants1.nih.gov/grants/forms.htm

A new pre-filled PHS398 cover page is available to download from our website at: http://www.dartmouth.edu/~osp/resources/forms.html

BE SURE YOU REPLACE ALL YOUR FORMS WITH THE NEW ONES DATED 04/06 NO LATER THAN MAY 10, 2006.

IN SHORT, the new forms will accommodate the changes in business process that NIH recently announced and were published in the NIH Guide.

They include in part:

  • the PI signature will no longer be required on the face page,
  • eRA Commons User Name is mandatory on the face page,
  • all efforts devoted to a project are reported in person months.

NOTE: the PI Assurance that was part of the cover page does not appear on the revised version. NIH will now rely on the institution to collect this compliance requirement from each PI upon proposal submission, and work is in progress to include the assurance language on the internal routing form.
