Revised PHS 2271, PHS 3734, and HHS 568 Forms Now
Available:
- "NIH announces the availability of three revised (11/2007) forms:
- PHS 2271 (Statement of Appointment);
- PHS 3734 (Official Statement Relinquishing Interest and Rights in a PHS
Research Grant);
- HHS 568 (Final Invention Statement and Certification)."

Revised PHS 2590 (DHHS Public Health Service Noncompeting
Continuation Progress Report) Now Available:
While most of the annual progress reports are
now submitted through the NIH Commons in electronic format, some award
mechanisms still require the annual report to be provided in paper format. Make
sure you use the latest version!!
"All progress reports received on or after
March 1, 2008 must use the new instructions and forms."

Appendices to Paper PHS 398 (DHHS Public Health Service Grant
Application) to be Submitted on CD:
"In an effort to further utilize
electronic technology, NIH is implementing the requirement that paper
applications provide appendix material only on CDs. Paper appendices will no
longer be accepted for applications submitted for May 25, 2008 and subsequent
due dates."
