1) Please tell us a little bit about how your program went. What was the format, how long did the program last, what was the goal/objective of your program?

2) How successful was your program? Please explain why you feel your program was successful or unsuccessful.

3) Please list all faculty members who attended your event.

4) What role did the faculty member(s) play in your program? (For example, did they facilitate a conversation? Did they take part in individual conversations with other students? Did they just observe?) Please explain their role(s) fully.
