Chargeback Form

This form is used in case of an overcharge in college billing

Da$h Fundraising Form   

This form is used to record signatures and student ID's at service events

Individual Da$h Fundraising Form

This form is used to record individual signatures and student ID's at larger service events (each page can be cut into 4 forms, one form per student)

Da$h Authorization Form 

This form should be completed by the student inputting Da$h at the GLOS office.Please email Ruth Kett to make an time to use the Da$h machine at 217 Collis.

Da$h Fines and Damages Form

Treasurers, presidents and / or House Managers may use this form to charge for damages or to fine members of the chapter. Please see the form for more details.
