Medical Records

Authorization forms to release medical information to any party (including College officials and insurance companies) are available from Medical Records at the Health Service Reception Desk, 5 Rope Ferry Road, Hanover or you may download a copy of the Authorization form. Your signature is required.

There is a $15 processing fee to cover the cost of photocopying and handling for full copies of Health Records.
