Medical Records

Authorization forms to release medical information to any party (including College officials and insurance companies) are available from Medical Records at the Health Service Reception Desk, 5 Rope Ferry Road, Hanover or you may  download a copy of the Authorization form. **IMPORTANT INFORMATION: NEW FAX NUMBER IS 603-646-9450. ** Your signature is required.

There is a processing fee to cover the cost of photocopying and handling. The fee is waived for copies to other health care providers.
