Don't forget your required pre-matriculation health requirements! Available at http://www.dartmouth.edu/~health/resources/incoming/. These need to be completed even if you waive DSGHP enrollment.
Dartmouth College requires all active students, including students not taking classes or on a leave term, to have health insurance meeting specified standards of coverage. To this end, all active Dartmouth students are automatically enrolled in the Dartmouth Student Group Health Plan (DSGHP) each September 1st, and the related fee is billed to their student account in August. To waive enrollment in the DSGHP, a student must submit a new, completed waiver petition each academic year. If a completed waiver application is received by August 1st, a student's enrollment in the DSGHP will be cancelled, and a credit will be given for the fee charged. Waiver Acknowledgements
All Insurance Waiver petitions will be acknowledged in two ways.
1. An email will be sent to the student's Dartmouth College email account known as Blitz mail and/or;
2. A Letter will be mailed to the student's permit address listed in Banner.
The deadline to submit completed waiver petitions is August 1, 2009, however, early waiver submission is highly recommended.

1 Dependent: $2,973.00
2 Or More Dependents: $4,171.00
NOTE: A new dependent application needs to be submitted for each plan year, dependent re-enrollment is not automatic. The deadline for enrolling dependents for the 2009-2010 plan year is September 1, 2009. However, we strongly recommend that you submit a completed DSGHP dependent application by August 1, 2009 to insure that coverage will be activated and ready to use on September 1, 2009.

The DSGHP benefit program is designed to complement services available through the Dartmouth College Health Service. Spouses and domestic partners covered in the DSGHP Family Plan may use the Primary Care services at Dick's House.
