Heart Failure Program

Our cardiology-based, multi-disciplinary program offers a comprehensive, specialized approach to the diagnosis and management of heart failure and cardiomyopathies. Services offered in both the inpatient and ambulatory care settings include:

• In-depth evaluation, diagnostic and therapeutic recommendations
• Optimization of Medical Therapy
• Advanced Heart Failure Management
  • Cardiac Resynchronization Therapy
  • IV inotropic or vasodilator therapies (inpatient, home infusions)
  • Evaluation for cardiac transplantation and mechanical cardiac support (collaborative program with New England Medical Center )
• Investigational drugs and devices
• Nurse Care Management Support
  • Telemanagement of heart failure symptoms
  • Patient education
  • Coordination of care, including home and social services
• Nutritional education and support
