Physician Connection Line
Whether an inpatient transfer, an outpatient consultation, a patient requiring a diagnostic test, or simply some telephone help in managing your patient, we hope to make the referral process as simple as possible. Depending upon your needs and the time of day, the referral will be facilitated using the following mechanisms:
Inpatient Transfers:
- During normal operating hours: Monday through Friday, 8am-5pm, calls regarding inpatient transfers should be directed to our CardiologyTriage Nurse, Roseanne Palmer. Roseanne can be reached either through the main DHMC number (603) 650-5000 or directly via pager (access pager menu via 603-650-8500 and enter pager 4668 for Rosanne). After a discussion about the clinical situation and needs (telemetry bed versus Coronary Care Unit), a plan will be determined and the transfer arranged as soon as possible.
- Note: In order to facilitate the transfer of the most acutely ill patients in an expeditious manner, DHMC has developed a list of patient clinical diagnoses which will lead to transfer on an emergent basis. These diagnoses include:
- Acute Coronary Syndrome
- Cardiogenic shock for IABP/LVAD
- Abdominal Aortic Aneurysm
- Massive GI bleed that requires TIPS
- Postpartum hemorrhage
- Dialysis pt with CHF or hyperkalemia
- Intracranial hemorrhage
- Scene trauma
- Organ transplant
- Thoracic aneurysm
- After normal operating hours and during weekends: Calls are generally handled by the on-call Cardiology fellow who can be reached through the main hospital number: (603) 650-5000. If the fellow is unavailable or if you feel the case complexity requires the involvement of the staff cardiologist on call, please feel free to request the staff physician when you call.

Outpatient Referrals:
- All outpatients cardiology referrals are handled by the Cardiology scheduling office at (603) 650-5724. Calls received after hours will be picked up by an answering machine and your office can expect a follow-up call early the next morning.
- While we intentionally leave openings in the schedule for urgent outpatient consultations, and most urgent patients can be seen within a day or two, you might sometimes feel that a patient is not being seen soon enough. If this is the case, please inquire directly to Roseanne Palmer, our triage nurse, who can be contacted via the mechanisms described above in "Inpatient Transfers".

Telephone Consultations:
- The cardiologists at DHMC are always happy to assist you over the phone with clinical issues that may not necessarily necessitate a formal visit or inpatient transfer. We are available by phone 24 hours a day. Contacting the cardiologist on call requires calling the cardiology desk during working hours ((603) 650-5700) or the main medical center number during evening hours and weekends ((603) 650-5000).
- If a DHMC cardiologist is already familiar with a patient with which you need assistance, we encourage you to attempt directly contacting the cardiologist by calling (603) 650-5700.
- We are always happy to help with your interpretation of an ECG. After calling us, tracings can be faxed to (603) 650-6164.

- This toll free number for referring physicians and their office staff is a single point of contact for services at DHMC. By calling this one number, you will be able to speak with a physician, schedule a patient for multiple appointments, and coordinate care for their patients.
