The 5th Annual Dartmouth Conference on Advances in Heart Failure

Monday, May 18, 2009

Dartmouth Hitchcock Medical Center

Dear Colleagues:

Heart Failure is a major cause of morbidity and mortality in adults affecting five million patients in the United States. Evidence based therapies have been shown to improve clinical outcomes. There is a critical need for physicians and nurses to understand both standard and emerging therapies as well as develop strategies of care delivery to help better manage these patients.

We would like to invite you to attend our 5th Annual Dartmouth Conference on Heart Failure: “Heart Failure- From Prevention to Intervention” to be held at Dartmouth-Hitchcock Medical Center, Lebanon, NH, on Monday, May 18, 2009. This year we have distinguished physician and nursing faculty who will focus on pathophysiology, prevention, heart failure with valvular heart disease, and special topics in hypertrophic cardiomyopathy and cancer-related heart disease. There will be ample time for discussion and collaboration. Break out work sessions have been designed to focus on the more practical aspects of heart failure management for physicians and nurses.

It is important for providers to understand not only timely diagnosis and evidence based therapy, but also the need to partner with all members of the health care team in order to provide optimal care. The complexity of heart failure diagnosis and management makes it a perfect prototypical chronic disease for a collaborative multidisciplinary approach.

This program should be of interest to cardiologists, cardiothoracic surgeons, internists/hospitalists, family practice and emergency room physicians, nurse practitioners, physician assistants, nurses and other health care providers interested in the care of heart failure patients.

We accept registration on-line at http://ccehs1.dartmouth-hitchcock.org, or by mail/fax. As in previous courses, we encourage you to register early as there is limited seating and enrollment.
