Your first appointment

Your first visit with a physician or medical center is often one preceeded by questions and concerns. These are some logistic matters you should consider before your first appointment:Depending on your health insurance, you may need a referral from your primary care physician in order for your appointment with your cardiologist to be covered. If your insurance does not require a referral, you can call us directly, (603) 650-5724. Please check with your insurance company to determine their policy.

Below is a checklist of information we’ll ask you to provide on your first visit.

The name of your primary care physician
Information about you:
  Name
  Birthdate
  Address
  Phone Number
  Insurance Information
  Your Complete Medical History and Record (If you are already a Dartmouth-Hitchcock patient, we will have this information in our electronic medical records.)
  Current Medications, including name, dose and frequency of administration.
  Allergies to medicines and other substances
