Ethics Concerns Form

Submitting a concern to the Business Ethics Helpline does not require completing all information, although if you choose not to provide contact information, please be as specific as possible regarding the nature of the concern. Raising such concerns is a service to the College and no one who raises a concern in good faith will be penalized. Submissions will be reviewed by the General Counsel and other persons with responsibility for the area in question.

The only required information are your comments, which you may enter at the bottom of this form. The other information is not required, should you wish to remain anonymous. However, omitting your information may preclude our ability to investigate fully and to assure you that we have addressed your request.

Dartmouth College respects your right to privacy. This form transfers your information securely and no information is saved that can be used to trace the individual or the computer used to submit this form, if you should choose to submit the information anonymously.

Guidance for submitting concerns: If you are writing to express a concern, specific details will assist us in the evaluation and resolution of the issue, such as: describing the issue, when it occurred and -- if it has occurred more than once -- how long and how often it has happened. Describe how you know about the issue. If you have notified the supervisor, describe what, if any, steps have been taken to address the issue.
