Health Assessment

Looking for an overview of your health status? Take your Health Assessment!

The Health Assessment is a confidential online health questionnaire that asks you questions about your lifestyle habits and well-being. It's available to all benefits eligible employees and only takes 15 – 20 minutes to complete.

Why should I complete the Health Assessment?

It's a quick and easy way to determine the current state of your overall health and to figure out what steps you need to take now to maintain or improve your health in the future.

How do I access the Health Assessment?

If you are an employee on the Dartmouth-sponsored Cigna health plan:

  1. Go to http://www.mycigna.com and log in or register.
  2. Select the Take my Health Assessment link on the homepage (just look for the red apple in the center of the screen).
  3. After selecting Take My Health Assessment you will be transferred to the my health & wellness center homepage where you can take the health assessment.

Read detailed instructions here.

Have questions? Read answers to frequently asked questions.

If you are a benefits eligible employee who is not covered under the Dartmouth-sponsored Cigna health plan:

  1. Go to https://dartmouth.mycareallies.com and sign in or click on "I need to register" and follow the registration instructions (you will need your Dartmouth ID-- if you're ID starts with a number, please don't include a 'd' in front of the number).
  2. Select the Take your Health Assessment quick link on the homepage.
  3. After selecting Take your Health Assessment you will be transferred to the my health & wellness center homepage where you can take the health assessment.

What happens after I complete the Health Assessment?

You'll receive a comprehensive Personal Health Report that includes suggestions and recommendations on ways you can maintain or improve your health that you can share with your Health Coach or doctor. You may also be invited to participate in online or telephonic health coaching programs to help support you.

You can re-take your Health Assessment at any time, so if you have been making positive lifestyle changes, take your Health Assessment again and receive an updated Personal Health Report!

Is the Health Assessment confidential?

Yes. Dartmouth College will not receive any individual results.
