Why should I complete the Health Assessment?

It's a quick and easy way to determine the current state of your overall health and to figure out what steps you need to take now to maintain or improve your health in the future.

How do I access the Health Assessment?

You can re-take your Health Assessment at any time, so if you have been making positive lifestyle changes, take your Health Assessment again and receive an updated Personal Health Report!

Is the Health Assessment confidential?
