Below you will find a summary of the near-term wellness activities that we encourage you to check out and spread the word about!

The three key wellness offerings, include:

Step 1: RECEIVE Your Biometric Screening: Know your numbers and become aware of your clinical risk factors. Although no more on-campus screening events are scheduled, there are still two ways to complete this important health activity and qualify for the $50* incentive:

  1. If you are a patient at Dartmouth Health Connect, receive your biometric health screenings at the practice no later than September 30th, 2013. The paperwork will be submitted on your behalf automatically.
  2. Receive your biometric health screenings with your own physician and submit the verification form no later than September 30th, 2013.

The screening will need to include:

  • Blood Pressure, both systolic and diastolic
  • Blood Glucose Level (fasting or non-fasting)
  • Lipid Profile, including total cholesterol and HDL
  • Body Mass Index Calculation (height-to-weight ratio)
  • Waist Circumference Measurement

**All active benefits eligible employees will receive a $50 incentive* for completing the biometric screening.

Step 2: COMPLETE Your Health Assessment

  • Spend 15-20 minutes online answering questions about your health. Based on information about your diet, activity, stress, and other factors you will receive a tailored comprehensive report with recommendations about your health.
    • For Cigna members:
      • Visit www.mycigna.com and register. Select "Take my Health Assessment" and follow the instructions.
    • For those who did not elect Cigna coverage (but are benefits eligible):
  • All active benefits eligible employees will receive a $100 Incentive* for completing the online questionnaire.

Step 3: ENGAGE with a Health Coach

  • Work with a Health Coach to set your own health and wellness goals

There are three ways to engage:

  1. Field Health Coaching:
    • Coaches are available at several locations. To schedule an appointment visit the www.dartmouthhealthcoaching.com or call 603.597.4033.
    • You must receive your biometric screenings prior to scheduling an appointment.
  2. Dartmouth Health Connect:
    • If you are a patient at Dartmouth Health Connect, you will receive coaching during your appointments at the practice.
  3. Telephonic Health Coaching:
    • Cigna members: Call 877.459.6150
    • Non-enrolled, benefits eligible employees: Call 855.561.3808
  • All active benefits eligible employees will receive a $50 Incentive* for completing two appointments.

* You must be an active benefits eligible employee in order to earn wellness incentives. Employees are considered benefits eligible when working at least twenty (20) hours each week, in positions of at least nine (9) months duration each year. Benefits eligible employees do not need to be enrolled in one of the Cigna health plans to be eligible for the incentives. All incentives will show up in your paycheck as extra money (minus the appropriate tax withholdings). Please allow approximately two pay periods to see the incentive.
