Wired Network Service Request

In order to process your request in a timely and efficient manner, you need to provide us with some basic information about your request or problem. We need the answers to all of the questions listed below in order to determine what the request/problem is prior to sending a technician out to fix it. This will ensure that the technician has the proper tools and goes to the proper location, thereby resulting in less time to process your request. This process may also help you find an answer yourself.

What is the name of your computer?

What is the IP address of your computer?

When was the last time you successfully accessed the wired network from your computer?

What is the number of the wall jack your computer is plugged in to?

Does this jack have a blue tab on it? Y N
(Only jacks with blue tabs are activated.)

If you plug your computer into a different jack [with a blue label], does it connect to the network? Y N

If you use a different Ethernet cable, does your computer connect successfully? Y N

If you have a Cisco IP phone, have you tried plugging your computer in to the additional Ethernet port on the bottom of your phone? Y N
If so, did this allow your computer to successfully access the wired network? Y N

If you want to use a jack that does not have a blue tab on it, that jack will need to be activated. Please activate jack #
Please note that we currently have very limited resources to activate additional jacks. In a few locations the infrastructure already exists which will allow us to activate additional jacks, and if the jack you requested is in one of those locations we will be able to fulfill your request.
