Forms and Approver Lists

Applications and a list of authorized approvers for departmental and individual sponsored DND or BlitzMail-only accounts:

Application for a Departmental BlitzMail Account - Used for requesting accounts for officially recognized departments, for accounts for sub-departments or groups within an officially recognized department, or for committees that are being created across departments. Must be signed by the major unit department head.
Application for a Sponsored DND or BlitzMail Account - Used for requesting accounts for people who are not, and will not, be paid directly by Dartmouth or are not Dartmouth students.
