Once again, congratulations on your admission to Dartmouth College! In order to ensure that your arrival at the College is smooth, the following information discusses how to complete the financial aid process. There is also a form that will provide you with the opportunity to request specific changes to your financial aid award. Although we try to accommodate your preferences in making adjustments, some requests are limited by federal and institutional policies. Each time your financial aid award is revised you will be sent notification via email to view the changes online.
We ask that you review the information, complete the form as required, and return it via email or fax to the Financial Aid Office by July 15, 2009.

Also discussed is how to complete a Master Promissory Note (MPN) with the lender of your choice. We have provided a comprehensive list of the lenders used by Dartmouth students and their parents within the last three years.
IV. Missing Information Follow-up
You can review a list of your missing or incomplete financial aid application materials on the Dartmouth College website using the instructions sent with your Admissions packet. Please submit any missing information to us as soon as possible to avoid delays in finalizing your award. Missing or incomplete documents will prevent funds from being credited to your student account and cause difficulty during the fall check-in process. If you have any questions about the information missing from your file, please contact us by e-mail at financial.aid@dartmouth.edu.
V. Student Employment Office
Information about Student Employment at Dartmouth will be provided in July and is available on the Student Employment website at: http://www.dartmouth.edu/~seo/.

Note: These forms are available in Adobe's PDF format. You will need the free Adobe Acrobat Reader software to view and print these forms. If you don't have Acrobat Reader installed, you can download it here.
I. Scholarship Data Sheet
This information is used to designate need-based scholarships through Dartmouth endowed scholarship funds to Dartmouth scholarship recipients who meet specific fund requirements.
II. Outside Scholarships and Awards
Please list any scholarships, grants, awards or prizes you have received from your community, school, church, employer, parents' employer or any other organization. These additional resources must be included as funding sources in your financial aid award. Your preference for award adjustment should be indicated on the form. Outside scholarships and awards will reduce or eliminate the loan and/or job in your financial aid package before Dartmouth Scholarship assistance is reduced. In some cases, these awards can also be used to cover the cost of your computer purchase, health insurance fee, or reduce the expected family contribution.
III. Computer Purchase
Information concerning the purchase of a personal computer has been sent to you by Dartmouth Computer Sales and Service. If you wish to apply for additional loan funding to purchase your computer, complete this section of the form. (Please note that loans are not available for printers or other peripheral equipment and loan funds are limited to the cost of the basic computer package.)
IV. Health Insurance Fee
Information about the Dartmouth College health insurance coverage requirement and the health insurance plan offered by Dartmouth has been sent to you by Dartmouth Health Services and is available online at: http://www.dartmouth.edu/~health/. If you would like to request funding to cover the cost of purchasing health insurance from Dartmouth, complete the appropriate section of the form. Students receiving Dartmouth scholarship assistance will automatically receive half of the billed insurance cost in additional scholarship.
V. Student Loan/Employment Adjustment Request
This section of the form should be completed and returned if you would like to decline your loan award or to request a change in the distribution of the self-help (job/loan) portion of your current financial aid award.
